About the role
An exciting opportunity is available for a Registered Nurse to join the Cardiac and Pulmonary Rehabilitation service at Glen Innes Community Health.
Working within a small, supportive multidisciplinary team, you will contribute to the delivery of high-quality, person-centred care to clients participating in Cardiac and Pulmonary Rehabilitation programs. The team prides itself on a strong culture of collaboration, teamwork and excellent communication, working together to achieve positive health outcomes for the community.
The successful applicant will provide individual assessments, conduct six-minute walk tests and facilitate small-group Cardiopulmonary Rehabilitation sessions incorporating exercise and education. You may also be required to provide the same service at Inverell Community Health.
About you
We are looking for a highly organised and motivated Registered Nurse who is confident working both independently and collaboratively. You will have the ability to manage individual client assessments while also facilitating small-group Cardiopulmonary Rehabilitation sessions in a professional and engaging manner.
You will work closely with the Physiotherapist to deliver the exercise and education components of the Cardiopulmonary Rehabilitation program, as well as collaborate with members of the broader Chronic Disease Team, including Diabetes, Respiratory and the Integrated Chronic Care for Aboriginal People Program.
What you will do
- Deliver individual Cardiac and Pulmonary Rehabilitation assessments and six-minute walk tests.
- Facilitate engaging small-group exercise and education sessions.
- Support clients to build confidence, improve health outcomes and self-manage their condition.
- Work collaboratively with the Physiotherapist and Chronic Disease Team to deliver coordinated care.
- Provide high-quality, person-centred care across Glen Innes and, when required, Inverell Community Health.
- Contribute to a supportive, collaborative and positive multidisciplinary team.

Essential Criteria
- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
