About the role
You will join an interdisciplinary team that aims to provide a high-quality specialist rehabilitation service to clients of the Midwestern Brain Injury Rehabilitation Program (MWBIRP)

This position provides a case management services to adults undertaking community rehabilitation with MWBIRP. This will include the assessment of client needs and the development and implementation of treatment programs for adult clients with cognitive, behavioural, psychological, communication and physical difficulties. Treatment programs could include both individual and group interventions.

The role of case management model d you would also be involved in co-ordinating the brain injury rehabilitation programs of a number of adult clients of the MWBIRP.

This position is a community-based role and will involve driving to see clients in their communities. Clients of the MWBIRP are usually residents in the following local government areas: Bathurst Region, City of Greater Lithgow, Oberon, Blayney, Orange City, Cabonne, Cowra, Weddin, Parkes, Forbes, parts of Lachlan Shire and Mid Western Region.

As a case manager you will be an integral part of the multidisciplinary health care team, providing high quality services to patients / clients of the WNSWLHD in order to meet their health and wellbeing needs.
