About the Program:

Hunter Valley Transitional Aged Care Program (TACP) is multi-disciplinary team supporting and providing aged care clients with the opportunity to improve their health and wellbeing after discharge from hospital. The TACP team comprises of Allied Health and Nursing professionals working closely with the clients and their family using an enablement focus, providing slow stream therapy aiming to improve the clients function, independence and safety. The service operates as a home visiting service, dependent upon patient needs. The service operates 8:00am - 4:30pm Monday - Sunday all year round. This role will be five 8-hour shifts per fortnight based at our Kurri Kurri office.


About the role:

As an Assistant in Nursing (AIN), you'll play a vital role in supporting clients to maintain their independence and quality of life within their own homes. This community-based role involves providing personal care and assistance with activities of daily living, tailored to each client's individual needs on the day.

Key responsibilities include:

  • Promoting medication adherence through gentle reminders

  • Providing basic wound care (e.g., simple dressings for skin tears), with more complex needs referred to an Enrolled or Registered Nurse

  • Supporting home exercise programs as prescribed by physiotherapists

  • Assisting with light domestic tasks, grocery shopping, and transport to medical appointments

You will work autonomously, visiting clients on your own. To support this, you’ll be provided with a work vehicle and mobile phone, and you’ll always have access to your manager or other team members for guidance when needed.

This role is ideal for someone who is confident working independently, enjoys building rapport with clients, and is passionate about providing compassionate, client-centred care in the community.

  • Certificate III Assistant in Nursing
  • A high level of autonomy and initiative

  • Excellent verbal and written communication skills

  • A genuine commitment to delivering safe, person-centred care
