What you'll be doing
Kids GPS Care Coordination is a network service designed for children with chronic and complex conditions who frequently utilise hospital services.
Kids GPS aims to improve efficiency and patient outcomes by supporting families to navigate services and providers within the hospital and the child’s local community, integrating tertiary, secondary and primary health. This enables patients and
families to access the right care at the right place at the right time by the right clinician.
The Appointment Coordinator will work collaboratively with the Kids GPS Care Coordinators, and other multidisciplinary clinicians to:
- Coordinate care between clinicians and services to minimise travel and the length of time spent away from home, off school and away from employment.
- Ensure all relevant information is well communicated between SCHN clinicians, local clinicians and the family.
- Provide advice and support to parents/carers regarding travel and accommodation options
Additional Benefits (for eligible employees): Flexible work opportunities, Salary packaging and meal entertainment benefits, accrued days off (ADOs), fitness passport, employee assistance program (EAP)
