An exciting opportunity exists for an Allied Health Care Coordinator (Level 3) with the Aboriginal Care Coordination Team (ACCT) at Sydney Children’s Hospital Randwick. This role provides care coordination for Aboriginal children and their families to manage their chronic conditions and link them into primary health care services. The Care Coordinator works closely with an Aboriginal Health Worker and Aboriginal Clinic staff who are based with the Community Child Health team. The ACCT Program is a joint initiative between The Sydney Children's Hospitals Network (SCHN) and Central and Eastern Sydney Primary Health Network (CESPHN) to increase care in the community for Aboriginal children. The Care Coordinator will aim to support access, health promotion and preventative health services to Aboriginal communities living within the CESPHN catchment area.
