About the role

Western NSW Local Health District (WNSWLHD) is seeking a dedicated Aged Care Outreach Pharmacist to join our multidisciplinary aged care team. This rewarding opportunity to make a meaningful difference in the lives of older patients across our region.

In this role, you will provide expert medication management and collaborate closely with nurses, geriatricians, allied health professionals, and community pharmacists to deliver, safe patient-centred care for older people.

A typical day involves reviewing patient medications, participating in case conferences, and working directly with patients and carers to ensure safe and effective treatment.
