Remuneration:  

·       Aboriginal Health Practitioner: $72,186 - $93,971 per annum

·       Aboriginal Health Worker: $65,012 - $93,971 per annum

·       Health Education Officer Graduate: $74,326 - $118,754 per annum

·       Health Education Officer Non Graduate: $64,985 - $93,974 per annum

·       Dietitian Level 1/2, Exercise Physiologist Level 1/2, Occupational Therapist Level 1/2 and Social Worker Level 1/2: $76,364 - $108,943 per annum

Location: Mount Druitt Community Health Centre, Western Area Adolescent Team

Where you'll be working
You will be based at Mount Druitt Community Health Centre, working within the Western Area Adolescent Team, a specialist youth health service committed to culturally safe, inclusive and accessible care for young people across Western Sydney. The team works in close partnership with Aboriginal communities, internal health services and external agencies to support young people experiencing vulnerability, marginalisation or complex health needs. You will be part of a supportive and values-driven environment that promotes collaboration, professional growth and culturally responsive practice.

What you'll be doing 
As an Aboriginal Youth Health Worker, you will play a vital role in improving access to health and support services for Aboriginal young people through culturally appropriate, client-centred practice.

You will:

·       Provide direct support to marginalised and at-risk young people through assertive case management, brief interventions, group work and health education

·       Build strong relationships with Aboriginal young people, families, communities and service providers to support continuity of care

·       Develop, implement and evaluate Aboriginal-specific early intervention and prevention programs and resources

·       Advocate for young people to ensure their health needs are heard and addressed within the health system

·       Participate in youth health intake, assessment, referral pathways and clinical supervision in line with best practice

Essential Criteria:

·       Qualification: Relevant qualification in social sciences, health or related field.

Selection Criteria: 

1.     This is an Identified Aboriginal/Torres Strait Islander Position. Applicants for this position must be of Aboriginal descent through parentage, identification as being Aboriginal and being accepted in the community as such. Exemption is claimed under Section 14 of the Anti-Discrimination Act 1977.

2.     Relevant qualifications in social sciences, health or related field and experience in the delivery of services to young people (aged 12-24 years).

3.     Demonstrated knowledge and understanding of Aboriginal and/or Torres Strait Islander cultures and have established links with the local Aboriginal community.

4.     Demonstrated understanding of health issues that affect Aboriginal communities and strategies to improve health outcomes for Aboriginal people.

5.     Demonstrated knowledge and / or experience in developing, implementing, reviewing, and evaluating culturally appropriate health education / health promotion / community development programs and initiatives to Aboriginal communities.

6.     Demonstrated ability to build and maintain successful partnerships with relevant partners including government and non-government organisations and community-based organisations.

7.     Demonstrated ability to work autonomously and as a member of a multidisciplinary team with excellent organisational, written and verbal communication skills.

8.     Current NSW class C drivers license and willing to use for work purposes.
