About the Role
Sydney Local Health District has a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.
The role of the Aboriginal Health Worker is to provide 48 hour follow up post discharge support from hospital for Aboriginal & Torres Strait Islander patients. This position is to ensure flexible, holistic and culturally sensitive health information on chronic diseases to our local Aboriginal and Torres Strait Islander communities.
Sydney Virtual is Australia's first Virtual Hospital and provides a personalised service for people in the community – a new way of caring. The Sydney Virtual care centre is able to connect with patients and their carers 24 hours a day, 7 days a week, by utilising technology to build on the legacy of expert staff, providing excellent patient and family centred care in the community.
Daily Requirements of the role include:
- Providing support to Aboriginal and Torres Strait Islander people upon discharge from a Sydney Local Health District hospital.
- Providing a culturally safe environment for patients to ask questions following their hospital admission and be aided with follow up of appointments and referral to other services.
- Assessment of patient wellbeing and confirmation of family support at home or in the community.
- Review of discharge plans with patient including medical appointments and medication supplies.
- Liaise with primary care and non-government services and link in with health services that provide ongoing care in line with patient’s goals and with consideration to specific cultural wishes.
- Validate concerns and increase health literacy for Aboriginal and Torres Strait Islander people while assisting Aboriginal and Torres Strait Islander people to navigate the health system.

Ideal Candidate
- Current NSW Drivers Licence (P1 and P2 acceptable).
- Hold or aspire to hold a minimum Certificate III qualification in Aboriginal Primary Health Care.
- Demonstrated involvement and 'lived experience' with an understanding and commitment to improving health issues in Aboriginal communities.
- A sound understanding of the cultural protocols with Men and Women's Business in a health setting.
- Experience in developing and maintaining strong stakeholder relationships, and confidence in negotiating with clinicians as part of the multidisciplinary team to advocate for best patient care.
- Knowledge and experience of health services and previous AHW experience is preferable.
