About the role

The Community Connector - Aboriginal Health Worker Transition to Practice role will work as a key member of a multi-disciplinary team. The Community Connector position will provide flexible, holistic and culturally sensitive information and support to clients to ensure they are sufficiently informed to empower them to make their own choices to access the relevant primary health care and supports that they require.  

The Community Connector - Aboriginal Health Worker Transition to Practice role will:  

  • Provide specialised and intensive assistance to help people within the Aboriginal community to understand and access health and other relevant supports in the community.
  • Use an assertive outreach approach to engage and build rapport with potential clients as well as local stakeholders and networks.
  • Connect with and support the Aboriginal and Torres Strait Islander Peoples and communities within the Dubbo Local Government Area (LGA).
  • Provide improved integration between the health and other systems at the local level.
  • Undertake training to upskill becoming a registered Aboriginal Health Practitioner.

The Community Connector - Aboriginal Health worker Transition to Practice position reports 
to and works closely with the Team Leader Aboriginal Community Connections and maintains a collaborative and productive working relationship with clients and stakeholders. The position is responsible for ensuring that all information and services provided are consistent with the values of NSW Health
