Where You’ll Be Working
You will join the Aboriginal Health Care Coordination Team, a supportive and culturally strong team committed to improving health outcomes for Aboriginal people and communities.
This role is based in Armidale and works across the surrounding communities of the New England region. You will work closely with Aboriginal clients, families, community organisations and health services to support culturally safe access to care.
The team values connection, collaboration and cultural knowledge, with weekly team huddles and monthly unit meetings to share updates, support one another and strengthen our work across the region.
About the Role
As an Aboriginal Health Worker/Practitioner, you will play an important role supporting Aboriginal people to access culturally safe, holistic health care.
Working as part of a multidisciplinary team, you will build trusted relationships with community members and partner services while supporting individuals and families to navigate health services and improve health outcomes.
Key aspects of the role include:
- Providing care coordination and individual support for Aboriginal clients and their families
- Engaging with local Aboriginal communities and organisations to strengthen partnerships and improve access to services
- Delivering health promotion and education activities that support community wellbeing
- Outreach across Armidale and surrounding communities, ensuring services reach people where they are
- Working collaboratively with clinicians and services to support culturally appropriate, holistic care

We are looking for someone who is passionate about supporting the health and wellbeing of Aboriginal people and communities.
You will bring:
- Strong connections to, or understanding of, Aboriginal communities
- Experience working with Aboriginal people, families and community services
- Knowledge of the local community and service landscape
- A commitment to culturally safe, respectful and person-centred care
- The ability to build trusting relationships and work collaboratively within a multidisciplinary team
Support and Development
You will be well supported in this role, with opportunities to grow your skills and develop your career in Aboriginal health.
Support includes:
- Opportunity and support to complete a Certificate IV in Aboriginal Primary Health Care (Practice) through TAFE NSW
- Comprehensive orientation, including face-to-face and online learning
- Ongoing mentorship and support from:
- Northern Cultural Team Leader (Aboriginal Health)
- Northern Clinical Nurse Consultant
- Operations Manager
- The wider Aboriginal Health Care Coordination Team
- Regular team huddles, peer support and networking with Aboriginal Health Workers and Aboriginal Health Practitioners across the HNE Northern region
Requirements
- This is an identified Aboriginal position. Applicants must be of Aboriginal and/or Torres Strait Islander descent in accordance with Section 14(d) of the Anti-Discrimination Act 1977.
- Willingness to work towards a Certificate IV in Aboriginal Primary Health Care (Practice) and Aboriginal Health Practitioner registration where applicable.
- Current NSW Driver’s Licence and willingness to travel within the local region.
