Remuneration: $58.83 - $62.51 per hour plus superannuation

The Aboriginal Health Care Coordinator (Integrated Team Care Program) is responsible for working collaboratively with patients and families, general practitioners, practice staff and Aboriginal health services to co-ordinate multidisciplinary care and services for Aboriginal and Torres Strait Islander people with chronic, long-term health challenges, within the guidelines of the Integrated Team Care program. Care coordination functions are carried out in accordance with the care plan developed by the patient’s GP.
The Care Coordinator will provide culturally appropriate care and advocate on behalf of Aboriginal and Torres Strait Islander patients.

Selection Criteria:
- Relevant clinical qualifications as an Allied Health professional with current AHPRA registration (where applicable) or as a Clinical Nurse Specialist 2 with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of the specified post-graduate qualification and AHPRA registration.
- Extensive post graduate clinical experience in chronic disease managementor community work with relevant clinical experience working with Aboriginal and Torres Strait Islander clients.
- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
- Demonstrated ability to provide high level clinical advice, education and support to others in providing service excellence.
- Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
- Demonstrated ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes.
- Current unrestricted NSW drivers licence (P2 licence acceptable).
