The Complex and Aged Care Navigator is a Clinical Nurse Specialist (CNS2) who is accountable for managing and supporting coordinated discharge planning for complex care needs patients and for ongoing care plans and appropriate discharge planning processes for Aged Care patients across Shoalhaven and David Berry Hospitals.

The purpose of the role of Complex Care and Aged Care Navigator is to:

• Work across multiple sectors with treating inpatient teams to assist in hospital avoidance and unscheduled representations of older people through the facilitation and support of clinical pathways for frailty, interventions, liaison and monitoring of all service providers who have ongoing roles in the care of older people.

• Work collaboratively with Emergency and other Departments to actively identify and manage older persons optimal care delivery including models of hospital avoidance, hospital substitution, sub-acute rehabilitation options and self-care.

• As part of a multi-disciplinary team be responsible for identifying and initiating discussion related to advanced care planning and end of life care where appropriate.

• Support nonhospital based care and nurse led service models and augment existing models of care internally to ISLHD facilities, and externally to GPs, RACFs and other providers.

• The Complex and Aged Care Nurse Navigator functions within C.O.R.E. values, the Policies and Procedures of the Department of Health, Shoalhaven and David Berry Hospitals and the Nursing and Midwifery Division.• Undertaking comprehensive assessment and care planning for inpatient older person with complex non acute care needs in addition to their acute or sub-acute care needs.

• Facilitating timely and effective discharge planning by providing information and assisting the inpatient older person and their person to contact to access services appropriate to their needs post hospitalization.

• Act as a resource and provide education to nursing staff regarding the unique and often complex issues associated with caring for older person (e.g. challenging behaviours, falls, dementia/delirium, etc.) and assist specifically with the minimization of non-clinical delays to their discharge.

• Assist the access of the older person and their person to contact to access appropriate post hospitalization aged care services as required. This includes, but is not limited to: Residential Aged Care, sub-acute care, Transitional Aged Care, Compacks, discharge support packages and home care packages.

• Participation in quality improvement projects and involvement with the National standards committee meetings
