The CNS will demonstrate the ability to act as a professional role model, delivering a high level capacity for client advocacy while demonstrating the ability to act as a change agent, provide effective clinical leadership and the ability to practice as an autonomous professional with the capacity to work within a Multidisciplinary team environment.
The CNS 2 will assist in the provision of supervision and support for less experienced assessors and will maintain their professional accountability and development in accordance with the Nursing and Midwifery Board Australia's Standards of Practice.
All staff are expected to act as an appropriate and effective role model and promote culture and supporting practices that reflect the NSW Health CORE values of Collaboration, Openness, Respect and Empowerment through demonstrated behaviours and interactions with patients, clients /carers and employees.
The CNS2 in ACAT will Undertake the key role is to provide comprehensive assessment, referral, support and information to patient/clients/carer of clients regarding services that require access to Australian Government funded aged care services, placement or alternate services community Aged Care packages including (CHSP services).
Provide a comprehensive assessment NSAF on My Aged Care, using mobile devises plan and co-ordinate care for clients.
Refer clients to any CHSP services when required in the community.
Provide information to patient/client/carer of clients regarding Commonwealth funded services.

· Current AHPRA Registration
