Where you'll be working
High Street Youth Health Service and the Western Area Adolescent Team provide specialised, youth-friendly healthcare services for young people aged 12 to 24 years. Our multidisciplinary teams work alongside vulnerable, marginalised and at-risk young people to improve access to healthcare, strengthen wellbeing and support positive life outcomes. What you'll be doing 
As a Youth Health Worker, you will work within a multidisciplinary team to engage, support and advocate for young people aged 12 to 24 years and their significant others. You will provide responsive, evidence-based services that improve access to healthcare and connect young people with the support they need to thrive.

Key responsibilities include:

·         Delivering assertive case management, brief interventions and care coordination within a primary healthcare framework

·         Building trusting relationships with young people and developing individualised care plans to address their health and wellbeing needs

·         Facilitating access to health, community and support services through effective care navigation and referral pathways

·         Designing and delivering health education, health promotion and group programs targeted to young people

·         Collaborating with internal and external stakeholders to strengthen service delivery, advocate for young people and improve health outcomes

Essential Criteria:

·         Qualification: Tertiary qualifications in relevant discipline

Selection Criteria: 

1.     Qualifications and/or experience in a relevant field. And Valid NSW drivers licence.

2.     Experience working with and understanding of the issues affecting marginalised young people’s access to health services.

3.     Demonstrated experience in planning, delivering and evaluating health promotion activities and/or community development projects.

4.     Knowledge of the health issues that impact on marginalised and at risk young people aged 12- 24 yrs.

5.     Excellent communication and networking skills.

6.     Demonstrated ability to work as part of a multidisciplinary team.

7.     Excellent computer skills with experience in a range of Microsoft applications.

8.     Demonstrated ability to engage with marginalised and at risk young people both individually and in a group setting.
