Wellbeing and Health In-Reach Nurse Coordinator
This position qualifies for a Recruitment Incentive Package up to the value of $10,000 (pro-rata part-time/temporary positions less than 18 months), and a reoccurring Annual Retention Bonus valued up to $10,000 (pro-rata part time), under the NSW Health Rural Health Workforce Incentive Scheme.
This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.
To recognise ongoing service in this role, once the successful candidate has served a qualifying period, they will become entitled to a reoccurring Annual Retention Bonus valued up to $10,000 (as taxable income).

What you'll be doing
Responsible for facilitating identification of health, wellbeing and social needs and the coordination of appropriate early intervention, assessments, and referral to services for school-aged children and young people attending designated primary and secondary schools.
- The WHIN Coordinator CNS2 is a Registered Nurse who works in identified local schools and applies their specialist clinical nursing knowledge, experience and skill in providing complex health care coordination for school aged children, young people and their families in these schools.
- The WHIN Coordinator CNS2 role requires leadership, specialist skills, capacity for a significant level of autonomy and independent work in an interagency context.
- Coordination of access to services across multiple agencies, professions and groups will be part of the role.

How to Apply
To be considered for this position, please ensure you address the selection criteria in the application as thoroughly as possible:
- Current Registered Nurse Division 1 National Registration (AHPRA) with a minimum of 3 years post registration clinical experience working with children and young people, a post registration qualification related to the specialty area of child and family health, or paediatrics, or adolescent / youth health nursing practice or other related area.
- Demonstrated understanding of and experience working with children and young people, including marginalised and at-risk groups, and health promotion, prevention and early intervention approaches and evidence based practice.
- Demonstrated capacity to engage with families to address various health and mental health problems for children and young people and effectively liaise with health and social services involved in their care.
- Demonstrated ability to work within an interagency context, with an emphasis on relationship building, service delivery, quality of care and team cohesion, and an ability to identify and improve opportunities to enhance integration of care for children, young people and families.
- Demonstrated excellent interpersonal and communication skills, including advanced verbal and written skills; an ability to negotiate and problem solve; and the capacity to collaborate with and influence health and other professionals.
- Demonstrated knowledge of and ability to utilise electronic communication and medical record systems across a variety of applications, including eHealth technologies.
- Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Ministry of Health Policies and National and State Standards in relation to children and young people.
- Current NSW State driver's licence (including P1 or above) and willingness to use for work purposes.
