About Your Role
The Transition of Care Pharmacist will facilitate continuity of care, optimise medication management and minimise the risk of medication misadventure for patient receiving transitions of care services, including patients receiving community care and hospital care at home.
The focus will be on supporting patients with respiratory conditions and heart failure during their transition home from hospital.
This role will be responsible for optimising medicines management and patient outcomes and reduce medicine-related harm during and after transitions of care. This will also involve some project and change management activities including setting goals, development and sharing of comprehensive care plans with patients/carers; enabling the delivery of medicine regimens and medicines management strategies that are safe, efficacious, cost-effective, and encourage supported and shared decision-making.
