About the Role
Community Mental Health Services provide a range of services designed to help support the recovery and help consumer stay well in the community. The team provide specialist services for young people, children and families, adults, and older people. Community mental health services include a specialist Aboriginal Mental Health Team, bilingual mental health workers and Peer Support Workers. Our main sites are at Canterbury, Croydon, Camperdown, Marrickville, and Redfern Community Health Services. This role will be based at Canterbury Hospital.

We are committed to developing the highest skills in our community mental health staff through the LHD’s pioneering Diploma of Mental Health program, structured workplace competency development program, and supervision programs.
The Assertive Outreach Teams are tertiary level community mental health teams providing care coordinated services to consumers diagnosed with a severe and enduring mental illness and who require an enhance level of support to sustain living in the community.
There are two Assertive Outreach Teams within Sydney Local Health District catchment:
1) The Assertive Outreach Team (AOT) based at Croydon Community Health Centre provides services to those living in Canterbury and Croydon catchment area.
2) The Mobile Assertive Treatment Team (MATT) based at Camperdown Community Health Centre provides services to consumer living in Camperdown, Marrickville, and Redfern catchment area.
The Team Leader of Mobile Assertive Treatment Team (MATT) / Assertive Outreach Team (AOT) is responsible for the management and coordination of an integrated multi-disciplinary recovery oriented mental health services. The Team Leader will manage the development of services and processes between Community Mental Health Services and non – government, primary care, social support, justice and drug and alcohol services to ensure collaboration, connectivity, reduce service duplication and ensure appropriate support is provided to consumers.

Ideal Candidate
- Recognised qualification in Nursing, Occupational Therapy, Social Work or Clinical Psychologist and current registration with AHPRA or eligibility for membership to the Australian Association of Social Workers.
- Extensive clinical experience in mental health, and health service management including demonstrated advanced level practice and supervision skills; service planning; implementation; and evaluation.
- Demonstrated leadership skills including implementing evidenced based planning and service evaluation relevant to the assertive outreach model of care and change management skills.
Understanding of and commitment to recovery-oriented services and consumer involvement in service design, implementation, and evaluation.
The preferred applicant will need to hold a Current NSW Drivers Licence (P1 and P2 acceptable).
