About the Opportunity
Are you a passionate and experienced Social Worker ready to take on a challenging and rewarding role in a high-acuity hospital setting? We are seeking a Level 3 Social Worker to join our dynamic multidisciplinary team on the 4D Renal Ward. This ward plays a critical role in hospital patient flow and supports individuals with complex clinical and psycho-social needs, including co-morbidities such as substance use, homelessness, trauma, and mental health conditions.

In this pivotal role, you will provide comprehensive psycho-social assessments, counselling interventions, and complex discharge planning to support patients through their hospital journey and beyond. You will work closely with the multidisciplinary team and external stakeholders to ensure safe, timely, and person-centred discharges. A key responsibility includes leading and facilitating applications to the National Disability Insurance Agency (NDIA) and coordinating access to community-based services. This is an opportunity to make a meaningful impact in the lives of vulnerable patients while contributing to the efficiency and effectiveness of hospital operations

 

What You'll be Doing
This position is responsible for providing specialist Social Work service to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

  

Where You'll Be Working 
Liverpool Hospital, founded in 1813, is the major health service for south-western Sydney, providing services to the local government area of Liverpool City Council as well as district services to residents and visitors in the area. It also provides a range of state-wide services in areas such as critical care and trauma, neonatal intensive care and brain injury rehabilitation.

The Hospital provides a health service of international standing, with 23 operating theatres, capacity for 877 beds, diagnostic and imaging services, emergency and trauma care, maternity, paediatric, cancer care, mental health, ambulatory care, allied health and medical and surgical services from birth to aged care.

Liverpool Hospital is currently undergoing a major redevelopment. The $790 million Liverpool Health and Academic Precinct (LHAP) project will provide enhanced facilities and an increased capacity to meet future significant population growth for South Western Sydney.

Liverpool Hospital is a principal referral and teaching Hospital of the UNSW Sydney and the Western Sydney University and also welcome students from over 20 universities and colleges.

Located in the heart of Liverpool City, the hospital is close to public transport, shopping centres and eateries.

  1. A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
  2. Extensive post graduate experience working as a Social Worker in relevant clinical areas, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.
  3. Demonstrated experience in prioritising a challenging and demanding workload; managing stressful situations; and applying advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature
  4. Experience working independently and collaboratively within a multi-disciplinary team.
  5. Demonstrated high level communication skills (written and verbal), and demonstrated computer skills.
  6. Demonstrated experience in the provision of clinical supervision and expert advice/consultation to the social work service and the clinical specialty.
  7. Demonstrated experience in the planning, implementing, leading and evaluating of quality improvement activities involving the application of evidenced based practice within social work
