The position is based at Redfern Community Health Centre. The role involves the provision of a community-based service across Sydney Local Health District. 

  

About the Opportunity

This is an exciting opportunity for a Social Worker to join the Chronic Disease Palliative Care multidisciplinary team. You are expected to provide a high quality Social Work service to patients/clients and carers in order to ensure the provision of optimal health outcomes for the patients/clients of the service.

Ideal Candidate

  • Must hold a Bachelor or post graduate degree in Social Work that provides eligibility for full membership of the Australian Association of Social Workers.
  • Ability to work as a member of a multidisciplinary team.
  • A current NSW Driver's Licence (P1 and P2 acceptable)
