Where you'll be working:

Gloucester Health Service is home to the Gloucester Soldiers’ Memorial Hospital – a 14-bed General Ward with a busy 3-bed Emergency Department and operating theatre offering General Surgery and Ophthalmology via Visiting Surgeons and Medical Officers. This position is based at the Community Health Centre, which provides a comprehensive range of nursing and allied health services to the local community.

Gloucester is the jewel of the Barrington Coast – a charming rural town nestled where the Barrington, Gloucester and Avon rivers meet. Just a few hours from Sydney and 1 hour 20 minutes from Newcastle Airport, this friendly town is surrounded by breathtaking national parks, rivers, waterfalls, and adventure opportunities. About the role:

As a Social Worker at Gloucester Health Service, you will play a vital role in providing high-quality assessment, treatment, and case coordination for clients. You will work independently while being supported by visiting clinical educators and remote colleagues, ensuring that patients receive the care they need in a collaborative, culturally sensitive and person-centred manner.

Key responsibilities include:

  • Providing client-centred clinical interventions as part of a multi-disciplinary team
  • Supporting patients and families, including navigating complex ward-based cases
  • Exercising independent professional judgement and working within organisational, legal and ethical boundaries
  • Contributing to quality improvement and enhanced consumer outcomes
  • Navigating systems such as aged care, NDIS, and guardianship processes
  • Delivering care that reflects best practice and aligns with evidence-based approaches

About you:

You are a compassionate and capable Social Worker who:

  • Is self-motivated and able to work independently
  • Brings strong communication and interpersonal skills
  • Can engage effectively with people from diverse cultural backgrounds
  • Has a solid work ethic, sound time management, and professional boundaries
  • Has experience with the Aged Care and NDIS systems
  • Understands (or is willing to learn) guardianship tribunal processes
  • Seeks continuous improvement and works collaboratively within a team



Requirements:

  • Relevant tertiary qualification in Social Work
  • Eligibility for membership with the Australian Association of Social Workers (AASW)
  • Experience in a healthcare or hospital-based social work setting
  • Understanding of aged care, disability, and mental health systems
  • Current NSW Driver’s Licence
