The Healthcare@Home Service Manager is responsible for the provision of strategic leadership and operational management of the district wide Healthcare@Home (H@H) service.

The position is responsible for the development, implementation, monitoring and evaluation of an integrated approach to care provided outside of the hospital setting.

The position will work closely with Illawarra Hospital Group (IHG) and Shoalhaven Hospital Group (SHG) and clinical service streams and departments. The position provides operational and strategic leadership for integrated, virtual, hospital in the home chronic disease care types within the H@H service.

The position will ensure the provision of high quality, evidence-based services for H@H patients. The role will ensure implementation as required of new service areas, initiatives and programs within H@H.

Primary Objectives of the role are:

          Effective day to day operations, service coordination and performance of H@H.

          Deliver high quality accessible care.

          Contribute as a member to the Out of Hospital Care service stream.

          Develop and encourage professional development of the multidisciplinary H@H service team.
