About the role
The Service Coordinator - Diabetes Education is a credentialled Diabetes Educator who will work collaboratively with the broader Chronic and Complex Care multidisciplinary services to provide a high quality, evidence-based diabetes service to the Orange community health and inpatient settings.
The role of the Service Coordinator is to provide key clinical leadership and to act as a resource person in the development, implementation, monitoring, reporting and delivery of diabetes services across Orange (and where applicable surrounding areas), contributing to patient care and working with the multidisciplinary team to ensure best outcomes for patients.
