In this rewarding role, you will make a meaningful impact on the lives of young children and their families by providing specialised support within a collaborative, community‑focused health team 

  • Primary and Community Health Social Work Department is seeking an experienced Senior Social Worker to support vulnerable families with children aged 0-5 in the Campbelltown and Camden LGA’s. 
  • This role supports the Sustaining NSW Families Child and Family Health Nursing team at Fairfield Community Health Centre. 
  • Sustaining NSW Families works with families identified as needing additional support based on the Support Families Early – Safe Start assessment guidelines. The role of the Senior Social Worker within this nursing team is diverse as it works with various internal and external stakeholders to provide service collaboration, case management and review. 
  • The Senior Social Worker is responsible for providing psychosocial interventions and support from a strengths-based model, providing holistic care within trauma informed, culturally safe interventions. Social Work ensures psychosocial supports and interventions are established in collaboration with the family and are adaptive to reflect their changing needs. 
  • In addition, the Senior Social Worker supports other Social Work clinicians, Child and Family Health Nurses and other Allied Health clinicians to ensure best outcomes for families.

To succeed in this Senior Social Worker role, you will bring compassionate, culturally responsive practice along with the skills and confidence to support families experiencing complex challenges. 

  • Experience working with families from CALD backgrounds, families with highly complex psychosocial vulnerabilities, and families with children aged 0–5 
  • Experience in navigating complex health systems 
  • High level of advocacy and case management skills 
  • Confidence in consulting with multidisciplinary teams 
  • Attention to detail, efficiency in documentation and care coordination  
 

Selection Criteria 
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below. 

  • A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
  • Can you describe a time when you managed a particularly complex case involving a patient with multiple health conditions? How did you apply advanced reasoning and independent judgement to navigate the situation, and what was the outcome?
  • Tell us about a quality improvement initiative you led within a multidisciplinary team. How did you incorporate evidence-based practice, and what impact did it have on service delivery or patient wellbeing?
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
