The Senior Social Worker, OOHC Health Pathways Program, is a permanent, part-time (32 hours), position located at the Caringbah Community Health Centre. The OOHC Health Pathways Program is a state-wide Model Pathway, which seeks to improve the health outcomes of children and young people in statutory OOHC.
The Role:
The OOHC Health Pathways Program is a state-wide Model Pathway, which seeks to improve the health Outcomes of children and young people in statutory OOHC.
This position will facilitate multidisciplinary health reviews of children and young people on the OoOH Health Pathways Program (HPP). Will plan, coordinate, and implement in-depth levels of clinical expertise, and extensive specialized knowledge within Social Work, in order to provide high quality trauma informed psychosocial screening and assessment of these Children and Young People which are consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and eligibility for membership the Australian Association of Social Workers.
- Extensive post graduate clinical experience in trauma informed psychosocial screening, assessment, and documentation of children and young people in OOHC or within a child protection context, with an understanding of the challenges experienced by Aboriginal, Torres Strait Islander, and culturally and linguistically diverse communities.
- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
- Ability to provide in-depth advice and education (to both internal and external partners) to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff if required.
- Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
- Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff including demonstrated experience with IT and data systems.
- Demonstrated commitment to ongoing clinical and professional learning.
- Current unrestricted Class C Driver’s license and a willingness to travel in accordance with the demands of the position.
