About the Role

As a Community Registered Nurse, you’ll have the opportunity to make a meaningful difference every day while working across a varied and rewarding caseload. You’ll provide high-quality, person-centred care in patients’ homes, helping them stay well, independent and connected to their community.
You’ll be involved in:

  • Managing your own caseload and coordinating care with patients, families and health professionals
  • Providing acute and complex wound care, including pressure injury management
  • Supporting continence, catheter and drain care
  • Delivering diabetes education and chronic disease management
  • Completing holistic assessments and individualised care plans
  • Working closely with community and palliative care teams

With every visit, you’ll have the chance to build trusted relationships, apply your clinical expertise and support patients to achieve the best possible health outcomes at home.

About You

We’re looking for an experienced and compassionate Registered Nurse who is confident working independently while being part of a supportive, collaborative team. You’ll be committed to delivering safe, high-quality, person-centred care and supporting patients to remain healthy and independent at home.
You’ll bring:

  • Current AHPRA registration and demonstrated Registered Nursing experience
  • Community nursing or relevant clinical experience, with strong clinical assessment, care planning and coordination skills
  • Excellent communication and organisational skills, with the ability to manage a diverse caseload
  • A positive, collaborative approach and a commitment to ongoing learning
