About the Service:
The Transitional Aged Care Program (TACP) provides short term care that aims to optimise the functioning and independence of older people after a hospital stay. It may include a package of services like low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care. It aims to enable older people to return home after a hospital stay rather than enter residential aged care.
About the role:
As a key member of the nursing team, you will undertake a range of vital responsibilities, including assisting clients with personal care, conducting clinical observations, delivering wound care, supporting activities of daily living (ADL) management, and facilitating home exercise programs.
You will work closely with a multidisciplinary team comprising nursing, physiotherapy, occupational therapy, podiatry, dietetics, and social work, all supported by comprehensive community services.
In this role, you will be a valued contributor, collaborating with the team to promote and maximise each client's independence.
We are seeking a highly skilled Registered Nurse who possesses:
- Experience providing general nursing care, including management of chronic diseases.
- Strong skills in wound care and personal care.
- Ability to support clients with activities of daily living (ADL) management.
- Experience with medication reconciliation and management.
- Previous experience working in community health settings is highly regarded.
Essential criteria:
- Current registration as a Registered Nurse with AHPRA.
- Eligible to drive in NSW with a current unrestricted NSW Drivers Licence
