Your experience and strengths
 

Working across homes, clinics, community settings and telehealth, you’ll use your clinical expertise and cultural knowledge to:

  • Provide health assessments, screening and preventative care.
  • Deliver health education, coaching and self-management support.
  • Coordinate care and connect clients with the right health and community services.
  • Support chronic disease prevention and management.
  • Provide follow-up care, yarning and culturally safe engagement.
  • Build health literacy, confidence and self-management.

Use your nursing expertise, cultural knowledge and passion for community care to build trusted relationships and create meaningful health outcomes for Aboriginal people, families and communities.

We’re looking for someone who brings:

  • Experience working with Aboriginal people, families and communities.
  • Knowledge and understanding of Aboriginal health across Hunter New England.
  • Strong communication, relationship-building and teamwork skills.
  • A culturally safe, person-centred approach to care.
  • Initiative, flexibility and confident problem-solving skills.
  • Experience in chronic disease management or care coordination (desirable).
     

Where you’ll work
Based at Adamstown, with flexibility to work remotely from Raymond Terrace Health Centre, you’ll work closely with the Nurse Manager as part of a collaborative team, with no direct reports.
