Where you'll be working:

Based at Tamworth Community Health Service, you’ll join the Integrated Chronic Care for Aboriginal People Program (ICCAPP)—a passionate, collaborative team working to close the health gap. Tamworth, in the heart of the New England region, offers a thriving community, rich cultural heritage, and an ideal balance of regional living with city conveniences.



About the role:

As a Registered Nurse in the ICCAPP team, you will work closely with Aboriginal and Torres Strait Islander people aged 15 years and over to improve access to culturally appropriate chronic disease care. Your role will be to support individuals and their families in managing chronic illness through flexible, person-centred, and culturally safe care delivered in partnership with local services.

You will:
  • Provide culturally responsive and compassionate care across community and post-discharge settings
  • Collaborate with Aboriginal Health Workers and local organisations to deliver coordinated care
  • Support self-management and long-term health outcomes for clients with or at risk of chronic conditions
  • Act as an advocate and build strong community relationships to ensure patients feel heard and supported
  • Participate in ongoing education and Aboriginal Health Unit engagement opportunities

  • Experience or interest in chronic disease prevention and management

  • The ability to build meaningful, trust-based relationships with patients and stakeholders



Requirements:
  • Current Authority to Practice as a Registered Nurse with AHPRA
  • Demonstrated knowledge and/or experience in chronic disease care
  • Current NSW driver’s licence and the ability to travel for community visits
