About the Role
This position is based at Concord Repatriation General Hospital. CRGH has a well-earned reputation for excellence in healthcare for over 70 years. Building on its proud heritage of caring for the Veteran community, the hospital now services the local communities of Concord, Strathfield, Burwood and beyond.
The successful applicant will have an interest in Patient Flow and work as part of the Demand Management team with the aim of meeting NSW Health Hospital Access Targets.
The Discharge Lounge Registered Nurse will be expected to utilise the Patient Flow Portal and work as part of the Demand Management team to identify patients for the Discharge Lounge prior to the day of discharge as well as the day of discharge
The Discharge Lounge Registered Nurse is responsible for ensuring the patients safe transfer of care to their discharge destination
The successful applicant will be working collaboratively with Bed Managers, Nurse Unit Managers, Medical staff, Pharmacy, Allied Health and Patient Transport Service
- The Registered Nurse is responsible for the provision of patient centred; evidence informed nursing care that promotes identified health outcomes. The Registered Nurse will assess, plan, implement and evaluate nursing care in collaboration with individuals and the multidisciplinary healthcare team.
Ideal Candidate
- Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA).
- Demonstrated understanding of evidence informed practice and critical thinking.
- Demonstrated commitment to person centred care and NSW Health Core Values.
