Where you’ll be working

You’ll be based at Forster Community Health, working as part of a highly skilled, multidisciplinary Community Nursing team. You’ll collaborate with services including Wound Management, Palliative Care, Cardiac and Respiratory Rehabilitation, Continence, Dietetics, Diabetes, Immunisation and Audiometry, and may at times provide support across the sector’s four Community Health Centres at Forster, Taree, Gloucester and Bulahdelah. This connected, community-based environment offers variety, strong teamwork and the chance to build meaningful relationships with older clients receiving CHSP-funded support.

About the role

As a Registered Nurse – Community Nurse (CHSP), you’ll provide a high standard of wound care in both clinic and home settings, supporting older clients to manage their health safely and independently. You’ll also complete catheter changes, port flushes and other community nursing interventions within the specialty clinic, tailoring care to individual needs while coordinating with the broader multidisciplinary team. At times you may work across the sector’s community health sites, giving you exposure to a diverse client group and service mix while remaining supported by experienced colleagues.

About you

You’re a competent, enthusiastic and team-oriented Registered Nurse who enjoys community-based care and is keen to further develop your skills in wound management and related community nursing specialties. You bring a collaborative mindset, strong interpersonal skills and a genuine commitment to supporting older people in their own homes and communities.

You bring:

  • Current AHPRA registration as a Registered Nurse
  • Current NSW Driver’s License and willingness to travel for work purposes
  • Ability to deliver evidence-based wound care in clinic and home settings
  • Willingness to perform catheter changes and port flushes within scope (with support as needed)
  • Team-focused approach and enthusiasm to learn within a multidisciplinary environment
  • Ambition to develop and extend community nursing skills aligned with CHSP and core service needs

These skills and attributes will help you thrive in this community nursing role, with support from the Nurse Unit Manager, guidance from a Wound Management Specialist, and education from the Clinical Nurse Educator, plus additional training opportunities aligned to the core business of the Community Nursing team.
