Join the Planned Care for Better Health team and use your nursing expertise to support people with complex health needs, improve health outcomes, and help reduce avoidable hospital presentations.
As a Registered Nurse within the Planned Care for Better Health Program, you will support consumers across the New England North West region by identifying those at risk of frequent emergency department presentations or hospitalisations. Through comprehensive file reviews, early intervention strategies, and effective care coordination, you will connect consumers with appropriate services to support their ongoing health and wellbeing.
Working collaboratively with healthcare providers and community services, you will promote consumer independence, support self-management of chronic and complex health conditions, and help ensure people receive the right care at the right time.
This predominantly office-based role provides the opportunity to apply your clinical knowledge, assessment skills, and nursing expertise in a proactive healthcare environment focused on improving outcomes for the community.
What You Will Bring
We are seeking a motivated Registered Nurse with strong communication skills, excellent organisational ability, and a passion for improving healthcare access and outcomes.
You will bring:
- Current Authority to Practise as a Registered Nurse with AHPRA
- Strong communication and networking skills
- Ability to build effective relationships with consumers, healthcare providers, and community services
- Sound clinical judgement, problem-solving skills, and attention to detail
- Confidence using digital health systems, including SDPR
- Ability to work collaboratively while managing priorities independently
