About the Role:

Western Newcastle Community Health Centre provides community nursing services to clients residing in the Western Newcastle Local Government Area (LGA). As a Registered Nurse, you will work as part of a multidisciplinary team to deliver holistic, person-centred care to clients and their families in their homes. This role involves the assessment, planning, implementation, and evaluation of evidence-based nursing care in collaboration with individuals and the broader healthcare team. You will build therapeutic and professional relationships with clients to support positive health outcomes in the community setting.

All staff are expected to act as effective role models, promoting a culture that reflects the NSW Health CORE values of Collaboration, Openness, Respect, and Empowerment through their behaviours and interactions with patients, clients, and colleagues. To support your transition into the role, an initial orientation period will be provided, including guidance from experienced mentors, Nurse Practitioners, and Clinical Nurse Specialists. Ongoing professional development opportunities will also be available through face-to-face training sessions.



The successful candidate will demonstrate the ability to:

  • Wound assessment and Clinical Management.
  • Generalist Management of PICC Lines, Portacaths and Clexane. 
  • Short term Diabetic follow-up post change in medication regime.
  • Specific therapeutic medication management.
  • Palliative care support in conjunction with other palliative care service providers.
  • General Nursing assessments and subsequent referral to other service providers.
  • Continence assessment and Clinical Management.
  • Urinary Catheter Care and Clinical Management.



Essential Requirements:

  • Current Authority to Practice as a Registered Nurse with AHPRA.
  • Eligible to drive in NSW with a current unrestricted NSW Drivers Licence.
