Join Inverell Community Health as a Registered Nurse within the Planned Care for Better Health Program and play a key role in delivering person-centred care for clients with complex and long-term health needs. The Opportunity

Reporting to the Nurse Unit Manager at Inverell Community Health, you will join a supportive multidisciplinary team delivering coordinated, person-centred care.
As part of the Planned Care for Better Health Program, you will provide health coaching, care coordination, care navigation, case management, and specialist nursing care for clients with complex and long-term health needs.
Working with teams across Inverell, Tamworth, and Newcastle, you will be supported by experienced nursing leaders while making a meaningful difference in the community.

Your Skills and Experience

To succeed in this role, you will be a skilled Registered Nurse with strong clinical judgement, excellent communication skills, and the ability to work autonomously within a multidisciplinary team. You will deliver person-centred health coaching, care coordination, and specialist support to improve outcomes and ensure clients receive timely, coordinated care.

You will bring:

  • Current Authority to Practise as a Registered Nurse with AHPRA
  • Experience supporting clients with complex health needs
  • Strong clinical assessment, communication, and care coordination skills
  • Ability to work autonomously and manage competing priorities
  • A current driver’s licence and willingness to travel for work purposes
