About the role:
The Residential Aged Care In-Reach (RAC-I) Service is part of the Hospital in the Home (HITH) team, delivering compassionate, integrated care that helps vulnerable aged care residents avoid unnecessary hospital presentations.
No two days are the same in this role. You’ll collaborate with multidisciplinary teams, liaise with acute hospitals and community services, and provide evidence-based, patient-centred care to residents and their families.
You’ll be actively involved in clinical decision-making, crisis response, advocacy, and supporting patients through complex care journeys.

You’ll be based in Maitland and work across the Greater Newcastle and Lower Hunter region. About you:
You are a capable, compassionate Physiotherapist with:

Requirements:
- Physiotherapist with current Authority to Practice with AHPRA
- Eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances.
