Allied Health Professionals Level 4: $63.81 - $65.36

Clinical Nurse Consultant Grade 2: $68.50 - $69.87

What you'll be doing
The NSRHS ORP Service aims to prevent re-fracture in patients aged 50 and over, post minimal trauma fracture, related to low bone density (osteoporosis). This is by ensuring a formal process for the early identification of patients at risk of future fracture, assessment of fracture risk, establishing goals of treatment based on assessment findings, development of multifactorial collaborative management plans, referral to external providers as required, and structured follow-up to ensure treatment goals have been achieved. The Fracture Liaison Coordinator is involved in the identification, assessment, care coordination and case management of patients attending the NSRHS ORP Services, under the clinical oversight of a nominated Physician (Rheumatologist, Endocrinologist or Rehab Physician).

Candidates will need to meet the following criteria:

  1. Registration with relevant professional body or eligibility for membership of the relevant professional body. A registered nurse with Authority to Practice with Australian Health Practitioner Regulation Agency and Minimum five years full time post graduate experience with 3 years full time equivalent in the specialty field. In addition must have an approved post graduate qualification (minimum Grad Diploma/Certificate level) or evidence of working towards Masters level qualification. Allied Health level 4 Physiotherapist, Dietician or OT will have recent extensive post graduate experience as a Level 2 or above.
  2. Demonstrated interest in musculoskeletal service delivery, including extensive knowledge of the contemporary issues around osteoporosis and its management.
  3. Demonstrated well developed understanding of effective models of chronic disease management.
  4. Demonstrated experience in co-ordinating multi-disciplinary care and the capacity to work cooperatively and effectively with medical, allied health professionals and consumers.
