Selection Criteria

  1. Current Registration as an Occupational Therapist with the Australian Health Practitioners Regulation Agency (AHPRA) or eligibility for accreditation as an Accredited Exercise Physiologist with Exercise and Sports Science Australia (ESSA).
  2. Demonstrated extensive knowledge and experience working with consumers with chronic and complex illnesses, including demonstrated ability to conduct advance care planning discussions.
  3. Demonstrated experience of care planning and case management of patients in the community with chronic and complex illnesses.
  4. Demonstrated understanding of specialist palliative care, a palliative approach and community health services.
  5. Demonstrated high level of interpersonal and communication skills, including written, verbal and computer skills and an ability to work collaboratively in a multidisciplinary team.
  6. Proven clinical leadership capabilities with the ability to provide consultancy advice, education and support to individuals, their families, allied health and nursing staff, and other health professionals.
  7. Current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances and overnight stays.
