About the role:
We’re looking for a compassionate and client-centred Occupational Therapist to join our Transitional Aged Care Program (TACP) based in Taree.

In this temporary part-time role, you’ll play a key part in supporting older people leaving hospital to safely transition back home - helping them regain independence and live well where they belong.

This isn’t a desk job. You’ll be out in the community, visiting clients in their homes, assessing their functional needs, and providing therapy that makes a tangible difference.

Working closely with the multidisciplinary TACP team, you’ll help set care goals, deliver interventions, and guide clients through short-term reablement with dignity and empathy.

In this role, you’ll:

  • Deliver community-based occupational therapy for aged care clients post-hospital discharge
  • Conduct in-home assessments and implement person-centred treatment plans
  • Collaborate with a multidisciplinary team to ensure safe transitions of care
  • Help clients improve function, achieve independence, and reach their personal goals
  • Support the overall success of the TACP program in line with Commonwealth guidelines


About you:
You’re more than clinically capable - you’re compassionate, flexible and know how to build rapport with older adults. You take pride in person-centred care, communicating effectively and adapting to client needs in real-world settings. Whether you’re new to the profession or bringing experience, what matters most is your commitment to quality care and team collaboration.

Requirements:

  • AHPRA registration as an Occupational Therapist
  • Demonstrated interest in aged care and community-based rehabilitation
  • Strong interpersonal, assessment and documentation skills
  • Confidence working independently in client homes and coordinating with a team
  • A current driver’s licence and access to reliable transport
