About the Role
The Transitional Nurse Practitioner Aged Care is an advanced practice nursing role delivering autonomous, high-quality, person-centred care across residential aged care facilities, community settings, and acute health services. Working collaboratively within multidisciplinary teams, the Nurse Practitioner provides comprehensive assessment, clinical decision making, diagnostic reasoning, and evidence-based therapeutic interventions to support older people with complex health and psychogeriatric needs.
This role is integral in reducing avoidable hospital presentations, supporting early discharge, and improving continuity of care through proactive management, home visits, and strong partnerships with Residential Aged Care Facilities (RACFs). The Nurse Practitioner demonstrates expert clinical leadership through mentoring, education, quality improvement, policy development, and research activities, while advocating for safe, innovative, and integrated models of care. Practicing within the Nursing and Midwifery Board of Australia Nurse Practitioner Standards Framework, the role promotes excellence in patient safety, clinical governance, and the delivery of holistic care that enhances health outcomes for older people and their families.
This is a Full-Time permanent role based at Port Macquarie Base Hospital.
About You
You are a highly experienced Registered Nurse with current registration through the Nursing and Midwifery Board of Australia and a strong commitment to advanced practice within aged care and psychogeriatric services. You are either endorsed as a Nurse Practitioner or actively progressing toward endorsement, with the capability and dedication to meeting all requirements within the designated timeframe.
With more than three years of extensive post-registration clinical experience, you will have developed advanced autonomous practice skills across aged care settings, with expertise in managing complex physical, cognitive, mental health, and psychosocial needs of older people. Your postgraduate qualifications in Aged Care/Psychogeriatrics underpin your comprehensive approach to assessment, clinical decision making, diagnostics, and evidence-based prescribing and therapeutic interventions.
You are recognised for your strong leadership, communication, and interpersonal skills, with a proven ability to mentor, educate, and support multidisciplinary teams while fostering collaborative and person-centred care. You are passionate about integrating current research and evidence into clinical practice to improve patient outcomes, drive quality improvement initiatives, and support safe, innovative models of care.
You are committed to delivering high quality, holistic care that promotes dignity, independence, and wellbeing for older people, while contributing positively to service development and patient safety initiatives and you hold a current NSW driver’s licence and am willing to travel to support service delivery across diverse healthcare settings.
