About the Opportunity

Join Murrumbidgee Local Health District and make a real difference in the lives of adults living with chronic and complex heart failure. We are seeking an experienced and compassionate Nurse Practitioner (NP/TNP) to provide specialist care across hospital, community, and virtual settings. This is your opportunity to deliver innovative, patient-centred care in a collaborative and supportive environment.

As the Chronic and Complex – Heart Failure NP/TNP, you will:

  • Assess, manage, and coordinate care for adults with heart failure
  • Support primary care providers and link patients with public services and private specialists
  • Use modern technologies to identify high-risk patients and provide virtual and face-to-face care
  • Lead a multidisciplinary approach in the community and collaborate with the Chronic Respiratory and Heart Failure Service, Integrated Care Service, and Community Nurses
  • Provide clinical guidance, mentorship, and education to colleagues across the region

About You

Our ideal candidate will demonstrate:

  • Advanced clinical expertise in chronic disease management, ideally in heart failure or cardiology
  • Authority to practice as a Nurse Practitioner or demonstrated evidence of eligibility to obtain authorisation within 2 years.
  • Ability to work independently while collaborating effectively with multidisciplinary teams across acute and community settings
  • Strong leadership, teaching, and mentorship skills, with experience in research and quality improvement
  • Excellent written, verbal, and interpersonal communication, including negotiation and computer literacy
  • Commitment to patient-centred, evidence-based care, and the use of modern technologies for virtual and face-to-face consultations
  • Flexibility to travel extensively across the MLHD region, with a current Class C driver’s licence
