About the Opportunity 
Step into a leadership role that makes a real difference! Join the dynamic SWSLHD Clinical Stream Women’s Health team and take the lead in driving the continued implementation of Pregnancy Connect across the Tiered Perinatal Network. This is an exciting opportunity for an influential midwifery leader to shape the future of care by collaborating and negotiating with midwives, consumers, and medical stakeholders to optimise consultation, streamline transfers, and support safe return journeys for women and babies with complex pregnancies. You’ll also have the chance to work closely with the Ministry of Health and Pregnancy Connect coordinators statewide, ensuring innovation and excellence in perinatal care.


What You'll be Doing
The Pregnancy Connect Care Coordinator is responsible for facilitating the planning, implementation and reporting to ensure integration of consultation, referral and transfer pathways across local health districts and within and across relevant Tiered Perinatal Network/s.

The Pregnancy Connect Care Coordinator will work in partnership with healthcare providers, including maternity and other health specialties, within their local health district and Tiered Perinatal Network, and the Ministry of Health to achieve the aims of Pregnancy Connect.

How to Apply

  1. Current midwifery registration with the Australian Health Practitioner Regulation Agency (AHPRA) with a minimum of three (3) years full time equivalent post midwifery qualification experience.
  2. Demonstrated understanding of the clinical and social needs of pregnant women and their families with complex care requirements.
  3. Demonstrated leadership skills and the ability to motivate, inspire and organise staff to achieve organisational outcomes.
  4. Well-developed analytical skills to analyse and interpret data, and apply insights practically to contemporary service delivery models, including development and evaluation of models of care.
  5. Demonstrated knowledge and understanding of principles and processes of project management, change management and awareness of issues that may occur as part of organisational change.
  6. High level written and verbal communication skills, with ability to write reports and correspondence, as well as the ability to design and conduct professional-level presentations, including for educational purposes.
  7. Demonstrated effective interpersonal skills, including the ability to forge partnerships at all levels both internal and external to the organisation, manage conflict and negotiate resolutions.
