We are seeking an experienced Clinical Nurse Specialist to join our IHG Access and Demand team as an Aged Care Navigator. As the successful candidate, you will play a key role in providing timely, high-quality discharge planning for aged care patients whom have complex needs. The role will focus on supporting decision making for aged care patients admitted to hospital, assisting with planning and seamless transition of care across our Illawarra Hospital Group facilities. Reporting to the IHG Access and Demand Nurse Manager, the role forms an integral part of senior decision making and expert care coordination.

Wollongong Hospital is the Illawarra Shoalhaven’s major referral and teaching hospital and has a bed-base count of more than 500. The campus, which incorporates the Illawarra Regional Cancer Care Centre, provides a comprehensive range of inpatient, outpatient and community-based services.
