As the Intake Registered Nurse for the Hospital in The Home (HiTH) service, you will serve as the initial point of contact for acute hospital referrals. Your role will involve clinically triaging referrals to determine urgency, suitability, and eligibility for home-based acute care. You will actively participate in daily multidisciplinary huddles to review service capacity, identify risks, and coordinate admissions and discharges to support safe and efficient patient flow.

You will work closely with HiTH medical officers, clinical leads, and hospital-based teams to facilitate seamless transitions of care. This role also requires direct communication with patients and their families to clearly explain the HiTH model, collect comprehensive clinical information, and ensure accurate documentation, follow-up, and care coordination.

In this position, you will be supported by the Nurse Manager, Nurse Unit Manager, and a broader multidisciplinary team. A structured orientation program will be provided, including shadowing experienced staff, system training, and a clear introduction to HiTH referral and triage processes. Ongoing professional development is strongly encouraged and supported through access to relevant external education in acute community care, triage, and virtual care models, as well as opportunities for postgraduate study, leadership development, and participation in clinical governance activities.

About You: 
 

  • Currently registered as a Registered Nurse with AHPRA

  • A proactive, adaptable, and resilient clinician with strong clinical reasoning and triage skills

  • Demonstrates excellent communication and interpersonal skills, with the ability to effectively engage with patients, families, and a range of healthcare stakeholders

  • Confident working autonomously while maintaining a collaborative, team-oriented approach, with strong organisational and time-management skills to manage complex care coordination

  • Passionate about delivering high-quality patient-centred care in community settings and committed to continuous professional development
