What you’ll be doing
As the Heart Failure Clinical Social Worker, you’ll provide comprehensive social work services to outpatients at the Heart Failure Clinic. You’ll support patients and carers facing complex psychosocial challenges that impact their cardiac health.

  • Conduct psychosocial assessments and provide tailored interventions

  • Deliver counselling, crisis support, advocacy and referrals to community resources

  • Work collaboratively with medical, nursing and allied health teams

  • Educate and support staff and students through supervision and training

  • Participate in supervision and professional development to maintain best practice

Essential Requirements:

  • Qualifications:  Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).

 Selection Criteria:

  1. Demonstrated core competency skills in psychosocial assessments and demonstrated understanding of issues associated with: Domestic violence, child protection matters, acute and chronic illness, age related illnesses, Guardianship and end of life matters.

  2. Demonstrated clinical experience or understanding of grief and loss counselling, crisis intervention, discharge planning and working with complex family dynamics.

  3. Demonstrated understanding of matters associated with working with people from Aboriginal and Torres Strait Islander background as well as people from culturally and Linguistically Diverse backgrounds.

  4. Demonstrated well-developed time management skills, excellent written and verbal communication, including report writing skills and high level computer literacy.

  5. Demonstrated ability to work with minimal supervision and as part of a multidisciplinary team

  6. A good knowledge of community resources and experience liaising with community organisations

  7. Commitment to quality improvement and evidence-based practice and a commitment to ongoing clinical and professional learning.

  8. Ability to work in a team providing 7-day rotating roster as well as participate in the After Hours On Call Social Work Service.
