About the Program

The Muswellbrook TACP team is part of the broader Hunter Valley Transitional Aged Care Program, working alongside colleagues based in Kurri Kurri and Singleton to deliver services across the Upper Hunter region.

The Transitional Aged Care Program (TACP) is a 12-week enablement program supporting clients aged 65 years and over (50 years and over for those who identify as Aboriginal or Torres Strait Islander) following discharge from hospital. The program focuses on restoring function and maximising independence, with a strong emphasis on collaboration, empowerment and individualised goal setting, enabling clients to safely transition back to living independently within their homes and community. About the Role

As an Enrolled Nurse within TACP Muswellbrook, you will play a vital role in supporting clients to return to their pre-admission level of function.

Key responsibilities include:

  • Providing personal care
  • Performing wound dressings
  • Overseeing prescribed exercise programs
  • Assessing and monitoring overall wellbeing
  • Occasional transport to local health appointments
  • Completing accurate documentation and facilitating required referrals

This is a community-based role requiring strong assessment skills, initiative and a genuine passion for enabling older people to achieve their goals.

We offer a comprehensive orientation to support your transition into the role, along with ongoing guidance from our experienced multidisciplinary team. You will receive dedicated support from the Clinical Nurse Educator (CNE) and Nurse Unit Manager (NUM), as well as education preparation for SDPR. About You

To be successful, you will have:

  • Current AHPRA registration as an Enrolled Nurse
  • Strong clinical and assessment skills
  • Excellent documentation and communication abilities
  • A collaborative, person-centred approach to care
  • A commitment to health promotion and enablement principles
