Where You’ll Be Working

The Transition to Residential Aged Care (TRAC) Unit is a 20-bed ward where acute care meets residential aged care. Patients who have recovered from their acute medical episode but are no longer safe to return home are supported through this transition, with nurses working closely with families and carers to ensure a smooth, compassionate handover into aged care facilities.

About the Role

As an Enrolled Nurse in the TRAC Unit, you’ll provide high-quality geriatric nursing care, including palliative care and social/emotional support, to patients during their transition into residential aged care. Your work will help patients and families navigate what is often a challenging and emotional change, ensuring care is delivered with dignity and compassion.

You’ll be supported from day one, with education and guidance available five days per week to help you develop confidence, skills, and clinical knowledge in aged care nursing. This is a rewarding role for nurses who enjoy building strong relationships with patients and families, working collaboratively within a supportive team, and making a meaningful impact in a unique care setting.

About You

You are a compassionate, motivated Enrolled Nurse with a genuine interest in aged care. You bring strong communication skills, a willingness to learn, and a team-focused approach to patient care.
