Where you’ll be working
The Transition to Residential Aged Care Unit (TRAC) is a unique 20-bed ward where acute care nursing meets residential aged care. Our patients have recovered from an acute medical episode but are no longer safe to return home. Here, we partner closely with families and carers to support patients as they transition into an Aged Care facility. It’s a service where compassionate nursing meets one of life’s biggest turning points.
About the role
As a TRAC nurse, you’ll provide geriatric clinical care, palliative support, and essential social and emotional care to patients and their loved ones. Every day brings meaningful connection as you guide families through what can be a difficult and deeply personal journey. This is more than nursing – it’s about being present, offering reassurance, and helping patients and families navigate change with dignity and compassion.
About you
If you bring the right attitude and a genuine passion for aged care nursing, we’d love to welcome you to our team. You are:
- Compassionate, adaptable, and committed to person-centred care.
- Eager to support patients and families through a life-changing transition.
- Open to learning and developing in aged care – junior nurses are strongly encouraged to apply.
How you’ll be supported
You won’t be doing it alone – our TRAC team has dedicated education support available five days a week to help you learn the ropes, build your skills, and develop confidence. With a team culture grounded in support, collaboration and growth, you’ll be encouraged to shape your career while making a profound impact on the lives of others.
