Multiple positions are vacant with job sharing opportunities, available at both Children's Hospital Westmead and Sydney Children's Hospital Randwick. Please indicate the preference for location of work for in your application. 

What you'll be doing
The Discharge Patient Flow Concierge Officer is responsible for supporting administrative processes for discharge planning and assists patients and their families with the process of leaving hospital and transitioning back to their home or other care setting.
 
The position holder will do this by supporting discharge processes in collaboration with the multidisciplinary team to expedite timely discharge and facilitate integration with out of hospital care and primary care services where appropriate.
 
This position is responsible for facilitating and enhancing communication between the patient, families, carers, clinical staff, out of hospital care services and primary care to improve the discharge experience for patients, carers and families.

  1. Experience in customer service in a health industry
  2. Experience in office management, organizational skills and customer service.
  3. Excellent interpersonal, communication and negotiation skills, in particular the ability to interact effectively with a multidisciplinary team.
  4. Experience working independently with minimal supervision
  5. Proven ability to work effectively within a multidisciplinary team to achieve key deliverables on time.
  6. Experience in problem solving and determine solutions
  7. Experience in high level IT skills and knowledge of Microsoft Office software, including Excel and proficient keyboard skills
