This is a Level 3 position. However, if there are no appropriate Level 3 applicants, applications from Level 1 and 2 will be considered. The selection criteria reflects the requirements for a Level 3 applicant. Responses from Level 1 or 2 applicants need to demonstrate their level of competence with regard to the same criteria and their ability to work towards Level 3. The expected performance of the incumbent will be commensurate with his/her years of experience and current level at commencement of appointment. The incumbent will work towards meeting the criteria for Level 3.
About the Service
Our Community Palliative Care Service provides specialist, person-centred care to people with life-limiting illness in their homes. We are committed to dignity, comfort, and quality of life — supporting clients and families with compassion and clinical excellence.

About the Role
This is a rewarding opportunity to deliver specialist occupational therapy services within a community palliative care setting.
In this role, you will:
Provide comprehensive functional and environmental assessments in clients’ homes
Prescribe and review equipment to maximise comfort, safety, and independence
Support symptom management strategies
Assist with complex discharge planning from acute services
Provide education and practical and emotional support to families and carers
Participate in case conferences and multidisciplinary care planning
Maintain accurate documentation and contribute to quality improvement initiatives
You’ll work autonomously in the community while remaining closely connected to a supportive multidisciplinary team. The role is varied and dynamic, requiring advanced clinical reasoning, adaptability, and confident independent decision-making.
Location:
Two days per week at Westlakes Community Health Centre
Three days per week at Neal House, Wallsend Campus
About You
You are an experienced and compassionate Occupational Therapist who brings:
Advanced clinical skills in palliative care and/or complex chronic care
Strong clinical reasoning and sound professional judgement in home-based settings
Excellent organisation and time management skills
The ability to manage competing priorities with resilience and flexibility
Outstanding communication and relationship-building skills
Confidence working both independently and collaboratively
A genuine commitment to dignity, client choice, and person-centred care
A passion for reflective practice, ongoing learning, and contributing to service development is highly valued.
