About the position:
As a Pharmacist Grade 1 you will assist in the provision of a comprehensive clinical pharmacy service in accordance with the SHPA Standards of Practice for Clinical Pharmacy Services and supports medication safety practices and quality improvement initiatives in accordance with National Standard 4.

Key deliverables:
Clinical: Conduct medication histories, reconciliation, chart reviews, medication management plans, and discharge counselling for patients/carers.
Education & Stewardship: Educate medical and nursing staff on NSW Health guidelines; support antimicrobial stewardship through antibiotic review and prescriber advice.
Documentation & Reporting: Record all clinical interventions in patient notes, IIMS, and pharmacy databases; submit reports to management and committees on time.
Quality & Professional: Participate in audits, KPIs, quality improvement, and relevant committees. Maintain AHPRA registration and support other MLHD sites as required.
Our ideal candidate will have:
Current AHPRA registration as a practising pharmacist
Demonstrated clinical pharmacy experience, with hospital experience or a postgraduate qualification highly regarded
The ability to exercise sound judgement, initiative, and independence in problem-solving and prioritising workload
Experience contributing to quality assurance and improvement activities
The ability to supervise pharmacy staff and students
A collaborative approach to working within a multidisciplinary team
A commitment to NSW Health's CORE values (Collaboration, Openness, Respect and Empowerment), patient safety, and compliance with relevant policies, legislation, and WHS requirements
Willingness to travel and undertake other duties as directed
