As a CNS2 level Wellbeing Nurse provides clinical leadership, have extensive health system knowledge, and operate with a high degree of autonomy and independence in an interagency context.
This role works in identified local primary and high schools, where they apply their knowledge and expertise to navigate health and social systems and provide integrated health care coordination.
This role's responsibilities include promoting the Wellbeing Nurse to school partners, engaging with stakeholders, and establishing robust consultation and referral networks.
This work allows the Wellbeing Nurse to provide high-level care coordination, ensuring access to a broad range of health and psychosocial supports across multiple agencies and professional disciplines.
This position will lead the health care coordination of referred school students and their families by facilitating the identification of health, wellbeing and social needs, and connecting clients to the most appropriate services for timely interventions and supports. Wellbeing Nurse activities are noted in the NSW Wellbeing Health In-reach Nurse Guidelines, which inform local implementation and guide in interagency care coordination.

- Current AHPRA Registration with relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate
